GENERAL ITEMS

9. APPROVAL OF SUPPLEMENTAL PLANT SURVEYS
Description

In 2011 the Florida Department of Education approved the district's 2011-2016 Five-Year School Plant Survey. Florida Statute 1010.3(1) (a) mandates that supplemental surveys be submitted when a school district seeks to remodel existing buildings or to construct new buildings not included in the Five-Year Survey. This survey amendment allows for updates to the survey recommendations for the Phoenix Academy buildings to become part of the Alta Vista Elementary School campus; Bay Haven School of Basics Plus construction of a new building for cafeteria, media center, stage, and classrooms; Sarasota High School building 5 renovations and demolition for classrooms, construction of a new gym and renovation of building 1 for a cafeteria and media center; Venice High to parcel out building 30 and 2 portables to be used by Sarasota Technical Institute for a Venice campus and North Port High to add a wrestling building. Upon approval by the Board, the submittal will be accomplished on-line through the state's EFIS network.

AGREEMENTS/CONTRACTS

10. AGREEMENT BETWEEN COMMUNITY REHAB ASSOCIATES, INC. AND THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA FOR PHYSICAL/OCCUPATIONAL/SPEECH THERAPY SERVICES FOR SCHOOL YEAR 2013-14
Description
Physical, occupational and speech therapy are related services required by both Federal and State law for students with disabilities.  Contracts with agencies such as Community Rehab Associates, Inc. assist in providing these necessary services when direct hire staff is not available.

Recommendation
That the agreement between Community Rehab Associates, Inc. and the School Board of Sarasota County, Florida, be approved as presented.

11. APPROVAL OF AGREEMENT FOR SERVICES BY INDEPENDENT CONTRACTOR BOYS AND GIRLS CLUB OF SARASOTA FOR THE 21ST CENTURY COMMUNITY LEARNING CENTERS IN NORTH COUNTY
Description
This agreement details the relationship and responsibilities shared by the School Board of Sarasota County and the Community Based Organization (CBO), Boys and Girls Club of Sarasota, to be responsible for the full implementation of the 21st Century Community Learning Center (CCLC) program in North County; service to be provided at six (6) school-based sites, CBO and FBO (Faith Based Organization) as detailed in the Grant.

That the School Board of Sarasota County approve the agreement with Boys and Girls Club of Sarasota to be responsilbe for implementing the 21st CCLC program in North County 2013-2014.

12. APPROVAL OF THE CONTRACT FOR SPEECH LANGUAGE PATHOLOGIST SERVICES BETWEEN KIM HARSHBARGER, INC AND THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA
Description

Ms. Harshbarger will be providing Speech and Language therapy and evaluation services for districted schools based upon student need.  These services are an IEP requirement.  The conversion of salary dollars will be used to cover this contract due to the lack of qualified applicants for Speech and Language positions.

That the contract between Kim Harshbarger, Inc. and the School Board of Sarasota County, Florida be approved as presented.

13. APPROVAL OF AGREEMENT FOR AFTER SCHOOL SNACK PROGRAM WITH SARASOTA YMCA AND THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA
Description
Sarasota County School Food and Nutrition Services will provide snacks for the YMCA after school program at the designated locations.

Recommendation
That the Sarasota County School Board approve the agreement for the snack program with the Sarasota YMCA as presented.

17. APPROVAL OF THE CONTRACT BETWEEN CHILDREN FIRST, INC. AND THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA
Description

This contract creates a mechanism to provide Early Head Start services for children and families in the areas of early childhood education, family partnerships, health, mental health, nutrition, and parent involvement.  Both Agencies will use specified resources to provide a comprehensive program for all children and families enrolled in the program.  Services will be provided at the North County Cyesis Program and the Cyesis Program at North Port High School (47 total Early Head Start slots: North Port; Riverview).

Children First, Inc. agrees to pay The School Board of Sarasota County, FL $130,660 in ten equal monthly payments to be paid August, 2013 through May, 2014.  Each monthly payment of $13,066 will be paid to The School Board of Sarasota County, FL by the 15th of each month.

GRANTS

18. APPROVAL OF THE EXPERT WRITING GRANT PROPOSAL TO THE EDUCATION FOUNDATION FOR VENICE MIDDLE SCHOOL
Description
COMPETITIVE: Grant funds of $4,700 will be used for activities to improve student writing.

19. APPROVAL OF THE LIVING CHEMISTRY GRANT PROPOSAL TO THE EDUCATION FOUNDATION FOR BROOKSIDE MIDDLE SCHOOL

COMPETITIVE: Grant funds of $5,000 will allow field experiences to improve middle school students' understanding of physical sciences.

Description
Bids to provide 'SCTI Marine Equipment and Supplies' were received from three vendors on August 15, 2013.  The bids of Integrated Supply Network and Mastry Engine Center, LLC, were the best low bids meeting the advertised specifications.  This bid is for a period of six months from date of award.  The funds for these purchases are contained in the capital budget allocated to the Sarasota County Technical Institute.

Recommendation
That the bids of Integrated Supply Network and Mastry Engine Center, LLC, to provide 'SCTI Marine Equipment and Supplies' in the amount of $53,839.03, be approved as presented.

26. APPROVAL TO AWARD RFP #14-0250 FOR LIFE AND LONG TERM DISABILITY INSURANCE
Description
Proposals to provide 'Life and Long Term Disability Insurance' were received from ten vendors on July 23, 2013.  The proposals of Minnesota Life for life insurance and Life Insurance Company of North America (Cigna) for long term disability insurance were the best proposals meeting the advertised specifications.  The proposal of Prudential Insurance Company of America is non-responsive to the requirement to submit a signed certification page.  The effective date of the contracts is January 1, 2014 and the rates are guaranteed for a minimum of three years.  The funds for these purchases are contained in the budget allocated to the Risk Management Department.

Recommendation

That the proposals of Minnesota Life to provide life insurance and Life Insurance Company of North America (Cigna) to provide long term disability insurance in an amount not to exceed $975,000.00, be approved as presented.

27. APPROVAL OF CLASSIFICATION OF INOPERATIVE EQUIPMENT AND FURNITURE AS PROPERTY TO BE DELETED
Description
The custodians of the property (cost center heads) have certified it to be traded in on new equipment, unaccounted for, not repairable or otherwise inoperable.  Property still on hand after reallocation will be salvaged for any useful parts and then sold at auction.   

Recommendation

Recommendation

Description

The Construction Services' Change Order(s) are summarized on the enclosure for ease of review.  These change orders collectively total a decrease of $187,898.51 in gross contract dollars.

Included in these change orders are $456,519.27 credits for Direct Material Purchases; the corresponding Direct Material Purchase Orders of $429,804.85 result a net of $26,714.42 in sales tax savings.

31. APPROVAL OF NON-EXCLUSIVE UTILITY EASEMENT AT THE SOUTH COUNTY BUS DEPOT BETWEEN THE SCHOOL BOARD OF SARASOTA COUNTY, FLORIDA AND FLORIDA POWER AND LIGHT COMPANY
Description
This easement will assign an easement for the construction, operation and maintenance of underground electric utility facilities (including wires, cables, conduits, and appurtenant equipment) to be installed from time to time; with the right to reconstruct, improve, add to, enlarge, change the voltage, as well as the size of and remove such facilities or any of them within an easement ten (10) feet in width.  This easement has been approved by the School Board attorney.

Recommendation
That the Non-Exclusive Utility Easement at the South County Bus Depot between The School Board of Sarasota County, Florida, and Florida Power and Light Company, be approved as presented.
